Rooted Life 

Rooted Life is based on the principles of Muladhara or the Root Chakra, which provides the foundation on which we build our life. It supports us in growing and feeling safe in all aspects of life. Rooted Life fully embraces these concepts. WE ARE ROOTED in supportive housing that encourages sustainable transformation where residents can learn to become integral members of their communities and families. 

Rooted Life focuses on progression, foundation and connection. We do our very best to meet people where they are, and encourage them to develop their skills and abilities, so they may live the best life possible.  By providing individualized attention and compassion as an essential aspect of our home, we can better assist those living with psychiatric vulnerabilities and addictions. Our teams provide an environment where men and women can move from surviving to living.

Our case management team is dedicated to championing the people who face the life challenges of mental illness and substance abuse. We partner with individuals and their loved ones to deliver an environment where participants can realize their full potential. By providing individualized attention and compassion as an essential aspect of our home, we can better assist those living with psychiatric vulnerabilities and addictions.

Support Group

Lead Care Managers

The overall goal of the Enhanced Care Management benefit is to provide comprehensive care and achieve better health outcomes for the highest-need beneficiaries in Medi-Cal. The Lead Care Manager (LCM) plays an integral part in providing and coordinating care, including assessing the client’s condition and providing them the necessary resources, options, and coordination for a healthy care outcome.

Lead Care Managers (LCM) are essential to the Enhanced Care Mnagement Process. LCMs:

  • Coordinate the delivery of services from various social service providers within the community to the program participants.

  • Assists program participants with Probation and Parole requirements, such as providing progress reports and supportive documentation about the program.

  • Conducts individual needs assessments, disseminates information, and assists the participants in understanding social service protocols and procedures.

  • ​Assist with coordination of medical and behavioral health appointments.

Hands Up

Resident Support Specialists

Resident Support Specialists manage Rooted Life facilities, ensuring that they are a safe, orderly, and nurturing space for our participants. This role involves a broad range of responsibilities from nutritional planning to maintaining safety and supporting the personal growth of each resident.

 

Resident Support Specialists (RSS) are essential to supporting the participants as they recover in Rooted Life facilites. RSS:

  • Meal Plan: Assessing residents’ dietary needs and preferences to plan healthy, balanced meals that suit various health conditions and cultural tastes..

  • Meal Prep Leader: Supervising or directly handling meal preparation, ensuring food safety and dietary compliance.

  • Facility Supervision: Overseeing daily operations of residential facilities, enforcing house rules, and keeping the environment clean and orderly.

  • Supportive Interaction: Providing ongoing support and guidance to residents to help them recover and develop personally.
